International Students (F-1) forms such as Sevis Transfer Form, I-20 extension form, and Curricular Practical Training can be download frrom here.

  • Sevis Transfer Form

Transfer students who have completed their AA or Program at TCC must fill out this form and submit it to the International Student Services Office, so their SEVIS record can be released to the new school. Download SEVIS Transfer Form (pdf 433.78 kB)

  • I-20 Extension Form

F-1 Students wishing to extend their I-20 must submit this form completed in order for your International Advisor to issue an I-20 extension.                            Download I-20 Extension Form (pdf 343.89 kB)

  • Curricular Practical Training (CPT)

F-1 students doing internships ON/OFF campus, paid or unpaid need CPT Authorization. F-1 student regulation permit an international student advisor to authorize employment for the purpose of CPT. Download CPT Form (pdf 866.55 kB)

  • Change of Status
