Post-Orientation Checklist

Submit Immigration Documents

Submit copies of your Immigration documents: Passport, F-1 Visa, signed I-20, and I-94

Proof of Health Insurance

Submit a copy of proof of purchased health insurance to ISS. You must be covered for every semester that you intend to study in the US.

Request from ISS the Supplemental Information Form and complete it.
