Applying for Services

Students with a diagnosed disability must apply for services through the Disability Support Services (DSS) Office before receiving accommodations.



​Complete the online Application for DSS Services. This application will provide information to your DSS counselor about what services you have used previously in educational settings. If you need the application in an alternate format, please let the DSS office know.


​Bring in a copy of your documentation. The documentation of your disability must provide your diagnosis, the evaluation that was completed to make the diagnosis, and the credentials of the physician or clinician who made the diagnosis.



​Make an appointment to meet with a DSS advisor. During this meeting, you will learn how to access accommodations on campus, what your rights are, and what your responsibilities are in receiving accommodations.
